New Primary Care Community Clinic in Scarborough Now Accepting Referrals

CMHA Toronto's Primary Care Community Clinic – Scarborough is
now accepting referrals. 

As part of CMHA Toronto's commitment to improving access to care and supporting overall health and well-being, this community-based clinic provides comprehensive, culturally responsive primary care for people of all ages who live in the following Scarborough postal code areas: M1B, M1R, M1W, M1S, M1H, and M1P 

The clinic is open to individuals who do not currently have a primary care provider. While the clinic is embedded within CMHA Toronto's community health infrastructure, people do not need to have mental health challenges to be eligible. The clinic is open to anyone who meets the eligibility criteria. 

Services Include 

  • Primary care and health assessments 
  • Preventive care and health promotion 
  • Chronic disease management 
  • Mental health and substance use-informed care 
  • Referrals and system navigation 
  • Health education and wellness support 
  • Connections to community and social services
